PETERSBURG, W.Va. — Former patients of Herrick Physical Therapy Services who need access to their previous medical records can request copies through WVU Medicine’s Release of Information department.
Following WVU Medicine Grant Memorial Hospital’s acquisition of Herrick Physical Therapy Services, patient records from the practice became part of WVU Medicine’s medical records system. Former patients may still request these records even if they do not have a WVU Medicine MyChart account or a WVU medical record number.
To request records, patients should contact WVU Medicine Release of Information using one of the following methods:
